Digital intake forms — Intake answers arrive complete, in your system, before the first visit

Digital intake replaces the clipboard, the PDF and the first phone interview with questions the client answers online. The questions change with each answer, so a divorce case and a car accident get different follow-ups. Answers are saved into the system staff already use, and anything urgent is flagged to a person.

Why it matters

The problem it solves

Paper and PDF intake forms come back with blanks, guesses and handwriting nobody can read. Staff then retype the answers, and every retyped field is a chance for a typo.

The bigger risk is timing. A court date, a drug allergy or a sick pet can sit on a clipboard until someone reads the form. Often that's mid-appointment.

Example · illustrative

A veterinary clinic's new patient

  1. On Sunday evening a new client books a Monday visit for her dog.
  2. She gets a text link and answers on her phone: species, age, vaccines and the reason for the visit.
  3. She writes that the dog has been vomiting since Saturday, and the form asks two follow-up questions about it.
  4. At 7:45 am the front desk sees the flag and moves the visit earlier.
  5. When she arrives, check-in is just confirming her address.

How it works

How digital intake forms works

  1. Starts whenA new client books, calls or asks to get started
  2. 1Step 1Send the intake linkThe client gets a link by text or email that opens on any phone, with no app or login.
  3. 2Step 2Ask by typeQuestions branch on earlier answers, such as case type, visit reason or pet species, so nobody fills in sections that don't apply.
  4. 3Step 3Check for gapsMissing required answers are asked again before the form can be sent, and dates and phone numbers are checked for format.
  5. 4Step 4File and flagAnswers are saved to the client record. Red flags, like a court date or a severe symptom, go straight to a person.
  6. A person decidesReview before the meetingStaff read a short summary and decide what happens next.

Before · by hand

  • Clients fill in a paper form in the waiting room, often with gaps.
  • Staff retype the answers into the practice or case system.
  • Missing details are found during the appointment itself.
  • Urgent facts sit on a form until someone reads it.

With digital intake forms

  • Clients answer from home, on their phone, before they arrive.
  • Answers go into the system without retyping.
  • Required questions can't be skipped.
  • Urgent answers reach a person as soon as they're given.

What we build

Your systems

  • Clio

  • Dentrix
  • Open Dental
  • Jotform
  • Google Forms

What we build

  1. 1Question set and branches

The questions for each client type, written with you, and the rules that decide which follow-ups appear after each answer.

  2. 2Mobile form or chat

A link that opens on any phone with no app or login. It saves progress so clients can finish later.

  3. 3Answer checks

Required answers, date and phone formats, and simple sense checks, like a birth date that can't be in the future.

  4. 4Red-flag rules

The answers that need a person right away, such as a hearing date or a severe symptom, and who gets told.

  5. 5System connection

Writes each answer into the matching field in your practice or case system, and attaches uploads and a PDF copy.

  6. 6Staff summary

A short readout of each intake, with flags at the top, for the person who meets the client first.

Your team decides

  • Staff review every intake before the first meeting and decide next steps.
  • An attorney, clinician or advisor decides whether to take the case or patient.
  • The business approves every question and every red-flag rule.

Industries it fits

What digital intake forms looks like in each industry

Law firms

Questions follow the case type, so a custody case and a car accident ask different things. Deadlines like a hearing date or a statute of limitations are flagged to the attorney.

Getting started

What you need first

We go through these with you in the audit, before anything is built.

Before we start

4 things to have ready
  • The intake forms you use today, paper or PDF, and a view on which questions you actually rely on.
  • Access to your practice or case system, or a decision on where answers land if it can't be connected.
  • Your red-flag rules: which answers need a person right away, and who that person is.
  • For patient data, tools covered by a BAA, and a decision on which staff can see intake answers.

How a rollout runs

  1. 1Map the questionsWe go through your current forms and first calls, drop questions nobody uses, and agree on the branches for each client type.
  2. 2Connect the systemWe match each answer to a field in your practice or case system and check that it lands in the right place.
  3. 3Test every branchWe fill in the form as different kinds of clients, including urgent ones, and check each flag reaches the right person.
  4. 4Go live for new clientsNew clients get the link, and staff compare each intake with what they learn in the first meeting.
  5. 5Refine the questionsQuestions that confuse clients or miss key facts are reworded, and new branches are added as services change.

What to measure

How to know it's working

Weekly report · digital intake forms

Illustration
  • Intakes completed before the visit

Shows whether clients get the link in time and can finish it on their own.

  • Forms started but not finished

Points to the question where clients give up, which is often one that's too long or too personal.

  • Fields corrected by staff

Shows which questions clients misread, and whether answers map to the right fields.

  • Time from red-flag answer to staff action

Urgent answers only help if a person sees them. This shows whether someone does.

  • Basics asked again in the first meeting

If staff still ask the same questions in person, the form is missing something.

We measure the baseline before building, so the change can be compared afterwards.

When it’s not a fit

  • Some clients won't use a phone form, such as some older patients or people without a data plan. Keep a paper or phone option, and decide who enters those.
  • A long form gets abandoned. If every question from every old form goes in, fewer clients finish, so cut questions first.
  • If the practice or case system has no API or import, answers may land as a PDF or spreadsheet that still needs filing.
  • Red-flag rules only catch what they're written for. A person still reads each intake before the meeting.

Tool or custom build

Do you need a custom build?

Option A · a ready-made tool

An off-the-shelf tool may be enough

Form tools like Jotform or IntakeQ, and the intake features in Clio Grow or many dental and veterinary systems, handle a standard form well. They're a good start if your questions rarely change.

Option B · built around your systems

When a custom build is worth it

A custom build makes sense when questions branch heavily by case type or answers must land in specific fields. It also fits when urgent answers must reach a person by text.

~$10–$40 / month

Fits when your systems, rules or exceptions don't match a ready tool.

Optional Automation Care: $125 · $250 · $375 / month

1, 2 or 3 hours of support a month, with monitoring and alerts included. Without it, you run it yourself after handover.

Typically works with Clio, MyCase, Dentrix, Open Dental.

Booking into or writing to your field-service, practice or shop software adds up to $500 to the ready-made tool (often a built-in feature) and $500–$1,500 to a custom build.

Your price depends on volume, the systems involved and how many exceptions need a person. Our team confirms a fixed price before any work starts.

FAQ

Questions about digital intake forms

  • Usually, yes. Most practice and case systems, like Clio, MyCase or Open Dental, accept data through a direct connection or an import. Where they don't, answers can land in a shared folder as a PDF.

  • Yes. Questions branch by case type, visit reason or service, so clients only see what applies to them.

  • Progress is saved, and a reminder can go out to finish it. Staff see who started and who hasn't.

  • Yes. Clients can take a photo with their phone, and the file is attached to their record.

  • It can be, when it runs on tools set up for that. For patient data we use vendors that sign a BAA, and access is limited to the staff who need it.

  • Yes. The form can be offered in the languages you choose. Free-text answers can be translated for staff, and the original is kept with the record.

  • Questions and branches can be edited as your process changes. Past intakes keep the questions that were asked at the time.

  • You do. The questions, answers and records stay in your own systems and accounts, and you can change or export them.
